Option 1: Case Review Consultation (30 Mins)


For specific patient cases:

  • Submit a PDF File of the patient information.

  • Include a signed Authorization to Release Information

  • Send to: cardinalacademy@cardinalpsychacademy.com

  • Must be received at least 24 hours before your appointment ( Case review sessions may be rescheduled if documents are not received at least 24 hours in advance)

Timing Tip: If you see a patient on Tuesday, send the packet the same day and schedule your consult for Thursday or later.

Option 2: General Consultation (No Patient Info Required)


Use this time to discuss clinical questions, treatment approaches, or complex cases (no patient info needed). If you have any questions, feel free to reach out.
